Performance Reset
$249
Two 60-minute visits: movement assessment, ForceFrame strength testing when appropriate, hands-on treatment when appropriate, home program, retest, and a written plan.
Or $125 per visit. Referral rate $199.
Plans
Your provider recommends how often to come in. You choose the session length.

Stay on Track: your plan renews automatically when one session is left and includes a full quarterly retest. Card on file required. Plans expire 90 days after purchase. Training twice a week? Use the 10-Week Plan over about 5 weeks.

Payment & policies
- Card, HSA/FSA, cash, or check. Superbills available on request; reimbursement not guaranteed.
- Payment is due before or at the time of service. Payment splits available on 10-Week Plans as listed.
- 24 hours' notice to cancel or reschedule. Late cancel: $30. No-show: $50.
- Plans are non-transferable. No refunds for completed services.
- Clients under 18 need a parent or guardian to sign all forms.
- Results vary and are not guaranteed.
